Aufbau eines Primärversorgungsnetzwerkes durch das Angebot eines Case- und Care Managements für Arztpraxen im ländlichen Raum

The aim of the project is to improve health care through case and care management in a rural region of three neighboring counties. In addition to the support of general practitioners, the medical, psychosocial and nursing aspects of treatment will be addressed by the case and care manager (CCM). The CCM will cover advisory, guiding and care-giving tasks in cooperation with family doctors and families. A joint concept is to be developed together with the medical caregivers, implementers, payers and other service providers. An accompanying evaluation of the case and care management is to take place.
