The Project "Improvement of cross-sector cooperation between general practitioners and clinicians in the care of oncological patients using the example of the comprehensive cancer centre CCC Tübingen-Stuttgart" aims to develop defined and practicable solutions for the improvement of cross-sectoral communication and cooperation using the example of the care of oncological patients of the CCC-TS. Especially for complex clinical pictures, in which several disciplines are involved in diagnosis and therapy planning, and which require at the same time very drastic therapy decisions, a long-term and continuous coordination between the actors is necessary.
