Stärkung der regionalen onkologischen Versorgung über optimierte Vernetzung mit Hausärzt*innen und Empowerment von Patient*innen

The aim of the OnkoNet project is to expand a regional network structure for the care of oncology patients. Within the network, communication pathways between oncology centers and general practitioners are to be improved, and needs-based services for patients and general practitioners will be developed (e.g. patient education, specialist advice from the oncology center). Patients should be able to access various services offered by the care network close to home.
After systematically assessing the needs of both general practitioners and patients, as well as their relatives, a central communication and competence platform will be established with the involvement of all project partners. Through this platform, network partners will use joint exchange formats and processes. The Oncology Center Karlsruhe will serve as the coordinator for regional/local oncological care and partner training. Initially, analog communication methods will be used, which will be supplemented and partially replaced by digital tools over the course of the project.
