Abstract / short description:
In the past decade, the introduction of new targeted and immunotherapy for metastatic melanoma led to a remarkable increase of patient outcome. However, treatment failure / resistance is observed in a remarkable proportion of these patients. Furthermore, immune-related adverse treatment effects occur frequently, often leading to treatment cessation.
Aim: In this project, we aim to identify quantitative radiomic tumor and extratumoral biomarkers for prediction of treatment response, patient survival and occurrence of adverse treatment effects in patients undergoing systemic state-of-the-art immunotherapy and targeted therapy for metastatic melanoma.
Method: A large patient cohort (n=572) has been prospectively maintained in our clinical melanoma registry. Demographic as well as baseline and follow-up clinical parameters of these patients are recorded. All patients underwent a prospectively standardized whole body CT staging at baseline as well as follow-up, additionally a subgroup (n=191) underwent PET/CT imaging using a prospectively standardized protocol. In all patients, radiomic properties of melanoma metastases and body composition will be extracted and assessed for their predictive capacity of treatment response, patient survival and adverse treatment effects. Finally, a statistical and a deep-learning approach will be applied to identify independent imaging biomarkers and assess their incremental predictive capacity to established clinical biomarkers.
